Any form need to fill after Payment in Kharagpur region (ACM-ICPC)

Only mail is Sufficient or do I need to fill any other required form in Kharagpur region? It’s written on FAQ Section to send a signed registration form which can be downloaded from http://acm.iitkgp.ac.in/ , but I’m not able to find any such form on their website .Screenshot%20(113)|690x434

You have to only send a mail with attached fee payement reciept.
I paid on 25th of sept. and now it is showing accepted for Kharagpur region.

How many days did it take for the team status to become accepted?

It usually takes less than a week.
